Your Personalized Relapse Prevention Plan

Before you leave, we build a continuing-care plan tailored to your life — so you step down with structure and support instead of a cliff's edge. Your plan may include:

Ongoing Therapy and Psychiatric Follow-Up

Appointments booked before you leave, not left as a task for your first week home. NIAAA sets out how stepping down through levels of care and staying in ongoing support normally works.

Relapse Prevention Tools

Your own warning signs written down, the situations that carry risk for you specifically, and a plan for what to do in the hour when it matters rather than in hindsight.

Referrals to Local and Virtual Resources

Named therapists, groups and prescribers near you, checked before we hand them over.

The weeks and months after treatment are when recovery is most fragile. Research on continuing care finds that staying actively engaged in support after treatment tends to improve outcomes, and that longer, more engaged continuing care generally does better than a short hand-off — see McKay’s review of continuing care in the NIAAA journal Alcohol Research. The effects are real rather than dramatic, which is exactly why aftercare is built into every stay here instead of being sold separately.
