Medical detox is the process of clearing substances from your body under medical supervision, with medications and support to manage withdrawal symptoms safely and comfortably. What to expect
How long that middle stage lasts depends mostly on the substance. These are general timelines, not a prediction for you — your assessment sets the real plan.
Symptoms usually begin within 6 to 24 hours of the last drink and peak over the first two to three days. A minority of people develop severe complications, including seizures and delirium tremens, which is why this is done under supervision rather than at home (Alcohol Withdrawal Syndrome, StatPearls).
With short-acting opioids, withdrawal typically starts within a day of the last dose and runs several days; longer-acting opioids start later and last longer. Opioid withdrawal is rarely life-threatening but is genuinely punishing, and medication makes a real difference to whether people stay the course.
Onset depends on the specific medication and can be delayed by several days. Benzodiazepine withdrawal carries a seizure risk and is tapered under medical supervision. Stopping abruptly on your own is the dangerous option here (TIP 45).
Cocaine and stimulant withdrawal is mostly psychological rather than physically dangerous — exhaustion, heavy sleep, low mood and strong cravings over days to weeks. Psychiatric support matters more than medication at this stage (TIP 45).
